Short Term Disability plans are designed to replace a portion of your monthly income while you are out of work for longer than 7 days due to an injury or illness.*

• You qualify for up to $3,000 per month for up to 3 months at a time
• Your doctor decides how long you can be out of work

Select from 10 year term or 20 year term. Rate stays the same during that time period. No underwriting questions.*

• Member: $50,000 benefit
• Spouse: $25,000 benefit
• Children: $10,000 benefit

BenExtend plans are designed to help ease financial stress of critical illness, accident or hospital stay with benefits for you, spouse and/or children for:*

• Hospital Admission Benefit
• Initial injury treatment including: ER, MedExpress, Urgent Care, Chiropractor
• Cash benefits for accidents such as: broken bones, lacerations, sprains, etc. 
• Pays a lump sum benefit for a covered critical illness such as cancer, heart attack or stroke
